Waiver, Release & Assumption of Risk

PART 1 - Rober Sport Karate Academy WAIVER, RELEASE & ASSUMPTION OF RISK

This agreement must be signed by all participants (or parent/guardian for minors) before participating in any Rober Sport Karate Academy class, event, camp, or activity. In consideration of being allowed to participate in Rober Sport Karate Academy programs conducted at or in connection with facilities including but not limited to the Alper JCC, I agree to the following:I understand that martial arts training involves physical contact and carries inherent risks, including but not limited to serious injury, permanent disability, illness, or death. I knowingly and voluntarily accept and assume all such risks, whether known or unknown, foreseeable or unforeseeable. I acknowledge that injuries may result from my own actions or inactions, those of other participants, instructors, or staff, the condition of the premises, or the use of equipment.


I certify that I (or my child) am in good physical condition and able to participate safely. I agree to inform instructors of any medical conditions or limitations. I agree to follow all rules, instructions, and safety guidelines provided by Rober Sport Karate Academy staff at all times. I voluntarily assume full responsibility for any injury, loss, or damage that may occur as a result of participation.


To the fullest extent permitted by Florida law, I hereby release, waive, discharge, and agree not to sue Rober Sport Karate Academy, its owners, instructors, staff, contractors, representatives, and affiliates, as well as the facility where activities take place, including but not limited to the Alper JCC, its directors, officers, employees, and agents, from any and all claims, liabilities, demands, damages, or causes of action arising out of or related to participation, including those caused in whole or in part by negligence.

I understand that Rober Sport Karate Academy operates as an independent program provider and is not owned or controlled by the facility where classes are held. I agree to indemnify and hold harmless Rober Sport Karate Academy and all listed parties from any claims brought by or on behalf of myself, my child, or any third party arising from participation. I authorize emergency medical treatment in the event of injury and understand that I am responsible for any associated costs.

Photo & Media Release

PART 2 - Rober Sport Karate Academy PHOTO & MEDIA RELEASE

I grant permission to Rober Sport Karate Academy to photograph or record me or my child during classes, events, or activities.

I authorize the use of such images or recordings for marketing, promotional, and educational purposes, including but not limited to websites, social media, advertisements, and printed materials, without compensation.

I waive any right to inspect or approve the final use and release Rober Sport Karate Academy from any claims related to such use.

I HAVE READ AND FULLY UNDERSTAND THIS WAIVER, RELEASE, AND ASSUMPTION OF RISK.

I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
I SIGN THIS AGREEMENT VOLUNTARILY AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE TO THE MAXIMUM EXTENT PERMITTED BY LAW.
